Why FUE or FUT Hair Transplants Don’t Prevent Hair Loss, and Hair Loss Management without Surgery

A 19-year-old gentleman wants to know whether he should get and FUE or the strip method. He wants to know what method has better results and is less damaging to the donor area.

Dr. Amiya Prasad, a hair transplant specialist says that there is a greater yield of hairs in the strip method versus the FUE, and all hair transplant surgeons agree on this. FUEs are almost the way a hair transplant was done in 90s but now they’re just smaller punches. Another disadvantage of FUEs is that by design, surgeons have to go outside the area of genetically resistant hair to retrieve grafts. People are not getting the density that they want out of transplants, lose a lot of hair from transection or cutting of the grafts, and not getting the yield of grafts they were hoping for. This has already been proven by the International Society for Hair Restoration that 65% of hair transplant patients want more density.

As someone who has lost so much hair at a young age, this gentleman has to think about what he would look like if hair loss continues years from now. The hair transplant area is limited and therefore the donor area cannot meet the amount of hair needed to cover the scalp.

Currently in the United States, finasteride and minoxidil are the two drugs used for male pattern hair loss. Finasteride is a drug that is taken orally. It essentially affects DHT or dihydrotestosterone sensitive hair follicles. If hairs are DHT sensitive, they will respond to finasteride. However, many people are concerned about the long-term sexual side effects and dysfunction related to taking finasteride. In Dr. Prasad’s practice, most men who come in are not interested in taking finasteride. Topical minoxidil is not going to stop the progression of hair loss. It can prolong the appearance of thinning hair on the scalp so it looks like there is more hair, but it will not reverse hair thinning.

Dr. Prasad would offer Hair Regeneration first, a method and formulation he developed using a combination of extracellular matrix(ECM) and platelet-rich plasma (PRP) to stop and reverse hair thinning wherever there is existing hair. This is very different from what is currently the standard of care for hair loss. For most male pattern hair loss patients, they learn about hair transplant and then they look for the best possible transplant. However, it is important to understand that no matter how old someone is, a hair transplant is a limited and temporary solution because patients will progressively lose hair. Many people don’t even consider a transplant because they don’t want a pluggy look. A majority of hair transplant patients with obvious plugs used to have more hair, but the problem was that the hair thinned was lost, causing the transplanted hair plugs to become obvious.

In Dr. Prasad’s practice, he does a microscopic evaluation of the scalp and identifies the potential hair that can thicken from Hair Regeneration. He can determine how much they would benefit from Hair Regeneration. He had patients who are very advanced in their hair loss and he has been absolutely impressed by the amount of hair volume they have been able to achieve in spite of how far along they were. He believes strongly that as long as there is potential, there is opportunity to get improvement with Hair Regeneration. He is able to show the benefit of Hair Regeneration as outweighing the benefits of hair transplant in terms of volume of hair. His strategy is to do first Hair Regeneration even if they are a candidate for transplant, allow 18 months to transpire and then see where they are at. With this onetime treatment, the benefit lasts for at least 3 to 5 years. If they choose a transplant, they can be more strategic, efficient and effective with the grafts if they have the Hair Regeneration treatment first.

Dr. Prasad is very hesitant to transplant someone this young. He would rather do Hair Regeneration, wait it out, watch and see if there’s any progression and then consider a transplant if there’s an appropriate understanding of what the limitations are.

      FUT does yield more grafts, which is why it’s often recommended, and progressive pattern hair loss may affect the temples first, but it won’t stop there if it’s untreated.       Sorry to hear about your problems. You may linking a cause and effect to your hair loss, as hair loss due to stress normally manifests in patches, and the hair does grow back. The Hair Regeneration treatment is primarily for treatment of genetic pattern hair loss, which occurs regardless of stress levels, and manifests with thinning of hair.       Yes, I have heard of post finasteride syndrome, which is common if you stop taking finasteride. Finasteride, if taken, needs to be taken daily and for life, or its effects wear off. If you don’t want to continue taking finasteride, look into the Hair Regeneration treatment in the video which actually stops and reverses hair thinning by making thinning hair grow back thicker. Finasteride only slows down hair thinning – it does not stop it completely nor does it thicken hair. Hair Regeneration has a success rate of over 99% in pattern hair loss patients, both in men and women, which is a higher success rate than finasteride. In fact, the patient in the thumbnail of this video was on finasteride in his “before” picture, but he responded remarkably well to Hair Regeneration. I hope the additional information helps you.

I tell my patients that the Hair Regeneration treatment lasts at least 3-5 years to manage expectations, but with that said, our original patients from over 5 years ago have not experienced hair thinning again.       Almost all hair transplant surgeons will agree that 1) an FUT gets more grafts than an FUE, but there are other reasons as well 2) FUT takes hair from the permanent zone, so hairs are not prone to thinning, unlike FUEs which can take hair outside of the permanent zone 3) FUT harvests hair grafts when the strip is outside the body, meaning it is easier to harvest grafts and keep them intact, unlike an FUE where grafts are harvested directly from your head, which has a higher incidence of damaging grafts also known as transection, which can be as high as 30% in FUEs 4) FUT is faster procedure for the doctor and the patient, meaning you will be more comfortable, and the chances of grafts surviving are higher since they do not spend too much time outside the body 5) With all its advantages, FUT is also costs less to perform as it doesn’t take as much time as an FUE 6) the majory drawback of the FUT is the donor strip scar. However, with expert closure of a cosmetic surgeon, the scar can be made virtually unseen. While the FUE’s main advantage is no large strip scar, it does make smaller scars throughout the head.

      Hair Regeneration is not a medicine that can be applied by anyone – it is a treatment that I would have to perform on you as it is an injection treatment, and I have to customize the formulation for each patient.       I’m sorry to hear about your problem. Unfortunately, Hair Regeneration does not work on traction alopecia as it is a condition where the hair follicle is damaged from tight pulling. The Hair Regeneration treatment works on dormant or inactive hair follicles that are still present. Traction alopecia would need a hair transplant to insert new hair follicles in the damaged areas.     1. You may have mistaken Hair Regeneration for another ACell+PRP treatment offered by another medical practice as they are not all the same. Most of our patients only have one treatment session done, which lasts at least 3-5 years. Some patients with advanced hair loss get a second treatment in the second year to build on the results of the first session, but few patients have a second injection. The first patients from over 5 years ago still haven’t experienced hair thinning again. I hope this clarifies things for you.

I had a 500 graft done to my left temple… now many have miniaturized …I wonder if I should go for an fue or hair regeneration therapy to cover the thinning area…may I send you a photo of my condition and have your advice thereafter.

      Hair Regeneration is not the same as PRP where you need a course of treatments – it’s a one-time treatment for most patients that lasts at least 3-5 years. Some patients with advanced hair loss may get a second injection in 18-24 months to build on initial results, but the vast majority of patients only need a single session. Cost varies with the extent of treatment, but at its full extent it costs about half of a single hair transplant session performed in the US. In the long run, it works much better, and costs a lot less than regular, ongoing PRP hair loss injections. I hope this clarifies things for you.

      There is an age limit for hair transplants, which are normally done after your 20s, but Hair Regeneration can be done in late teens or 20s since it is not surgery, and it does prevent progressive hair loss.
